All product must be pre-ordered prior to March 23, 2009 to ensure availability at the event. To help process the orders efficiently, we would request an individual form (click here for a 2-page PDF or scroll to the bottom of this webpage for seperate PDF pages) be completed by each of the interested members of your team, and in turn they are passed on to their respective Manager to allow a team order to be processed. Ideally all orders should be combined into one team order. If necessary, please fax all individual orders from the team at one time, clearly indicating the Team Name and Manager.
